Nice practice sheet. Useful for all level of chemistry if the error is fixed.

Technical Remarks:

The number of neutrons can not be calculated from the information given. The masses shown are average masses. A similar way would be to leave out symbol and name, but give protons or Z and let them fill in the rest.
