I have been using VTrain (Vocabulary Trainer) for weeks in my EFL language lab. This software is a quite successful attempt to implement flashcard-based learning on a computer. It uses Question & Answer flashcards.
The most remarkable feature of the testing module of VTrain is that it can draw up a schedule of repetitions for each flashcard. This way, each card will need to be reviewed only a few times before it enters your long-term memory.
Using this software, my students are now learning vocabulary on their own, so there is more time left to focus on grammar in the classroom.
The software also includes an editor along with the testing module. VTrain flashcards can contain multimedia elements, and remarks and hints on the cards can be commented out.
The authors claim that more than 20 colleges have registered VTrain. A small figure, considering its many benefits.

Technical Remarks:

VTrain's interface.- Coming to terms with VTrain may prove a hindrance to unmotivated users. The program contains a nice tutorial that explains the basics. Make sure your students go through it carefully!
Using VTrain over a LAN.- If you install a copy of VTrain on each computer connected to your school LAN (the download page includes a step-by-step explanation to this), you will, as a teacher, be able to modify and correct your flashcard collections at any time. Changes will be mirrored on each of the students' computers.
